Case Study: Clinical AI Diligence

Client Context: A mid-stage venture firm was evaluating an investment in a clinical decision support platform using generative models to draft Treatment Plans and Prior Authorization requests for oncology clinics.

The Challenge: The target company boasted high model accuracy and clinician approval rates, but lacked structural boundaries, formal override mechanisms, and traceability for system failures. The investor required a rigorous sociotechnical risk profile prior to closing the round.

1. System Evaluation & Scope

Ethotechnics conducted an independent, 2-week diligence review of the target’s product architecture, clinician feedback loops, and fallback pathways. We assessed the system against three core safety criteria:

  1. Temporal Boundaries (STD-01 compliance): Does the system enforce clocks on draft expirations?
  2. Override Authority (STD-02 compliance): Can clinical users instantly pause or reject automated plans?
  3. Escalation Latency: Is the time-to-detect model drift shorter than the time-to-impact?

2. Findings & Identified Risks

Our safety review identified two high-impact vulnerabilities:

  • Silent Claim Drift: Model updates altered oncology treatment recommendations without triggering code/registry version updates. Clinical users assumed the underlying guidelines were static.
  • Escalation Friction: Clinicians wanting to override an incorrect Treatment Plan had to open a separate support ticket, introducing significant cognitive and operational friction.

3. Remediation Roadmap

We provided the investor and target company with a concrete, 30-day safeguards mitigation roadmap:

  • Draft Expiration Rules: Embedded strict draft clocks on all Treatment Plans so stale model outputs auto-expire.
  • Inline Override Interface: Replaced the separate support ticket workflow with an inline "Flag & Pause" control, satisfying STD-02 override standards.
  • Telemetry Monitors: Integrated live drift monitoring linked to active clinician reject signals.

Outcome: The investor closed the round with custom safety covenants in the term sheet, and the target team successfully integrated the override safeguards within 3 weeks.
